Transition Planning

Transition Planning is a formalized deliberate, facilitated approach developed for any organization that needs to migrate its organization from its current state to some known and defined desired future state. The developed plan includes capture of all key elements of the future state organization as well as the roadmap for transitioning from the current state to that future state.
